A 3-piece penile implant has two cylinders, a pump and a reservoir. When the pump is used, fluid is transferred between the reservoir and the cylinders. The 2 cylinders are placed in the Corpus Cavernosa, the pump is placed in the scrotum and the Saline Reservoir is placed inside the abdominal wall. You have to press the pump situated in the scrotum between the two testicles several times to get the maximum erection.
The erection would stay for as long as you want and once the need for erection is over you just have to bend the penis shaft to make it flaccid. When you bend the penis, the fluid in the cylinders goes back into the reservoir and the cylinders become empty thereby making the penis flaccid. It is not possible for anyone to find out that you have an implant inside your penis unless they see you pressing the pump in the scrotum.
It has been found in numerous surveys that 3 Piece Penile Implant results in greater patient satisfaction than 2 Piece Implant. It has been seen that for penis which is longer than 6 inches, a 3 piece implant would give more rigidity as well as more flaccidity. The reason for this is that the fluid in the 3 piece implant is stored in the reservoir and not in the cylinders.
The reservoir can have more fluid and that is why longer penis would do better with a 3 piece implant than with a two piece. However for most people the penis shaft is less than 6 inches in length and therefore the results of both the 2 piece and 3 piece implants would be same in terms of rigidity. However since the two piece implant stores some of the fluid in the cylinder so the base of the penis would be slightly rigid even when the penis is deflated in case of two piece implant.
